If a page already has a rotation set, does rotating it again add to that or replace it?

It adds to it. PDF rotation is stored as an absolute value per page, so applying another 90° rotation composes with whatever was already there — for example, a page already stored at 90° becomes 180° after one more 90° rotation, not 90°. The "Total rotation applied so far" shown after each rotation reflects this combined value.

Will the rotation show up correctly if I print the PDF or open it elsewhere?

Yes — rotation is stored using the PDF format's own standard rotation setting, which virtually every PDF viewer and printer already respects automatically. The page's underlying dimensions aren't changed, only how it's displayed.
